ABOUT YOUR VISIT
Preparation for your appointment
Please come to your dental appointment on time. You do not need to bring your insurance card if you do not have one. However, we check IDs during new patient appointments. We will update your medical information with you electronically once you are in the office. Please bring a list of medications you are taking. If you are seeing a specialist, such as a cardiologist, for a medical condition please have their name(s) and contact information available.
Our staff speaks Spanish, Urdu, Korean, Serbo-Croatian, Hindi, and Persian. Please accompany any patients 17 years old or younger to their appointments, or we will not be able to treat them. Thanks for your cooperation and see you soon!
