APEX DENTAL CARE

We want to make filling out forms a breeze! Our goal is to keep your care safe and make sure we have all the information we need prior to your visit without making you do a ton of paperwork. We are excited to partner with Modento to help us achieve this goal. Forms will be available in Spanish as well. 

To provide our patients with the most individualized care, as of 02/17/2022, customized forms will be sent via link to patients prior to the their appointment.  If you are not comfortable with digital forms, no worries! Ask for paper forms in the office.
